Details:	SUMMARY: Re-opening DETAILS: Jim Solvedt or Jason Borzy of Polk County will fill out inventory forms and submit the coliform and nitrate results when they receive them. In so doing, the Department will re-activate the water system. ACTION NEEDED: Setup monitoring schedule and reactivate WS upon receipt of inventory forms, coliform & nitrate results, and well log or SWA showing the 1950 well meets current construction standards.
