Details:	SUMMARY: Coliform routine samples DETAILS: Alyson Reynolds had a conversation with Paul VanDehey regarding the raw sample tap at the well head (see contact report 8/17/09) I spoke with Chris and she mentioned she had not seen a raw sample tap during any of her visits to the site. She also indicated that she had only been in the pump house. Our records indicate that the wellhead is located to the west of the pump house in a 25" deep pit covered by a manhole and contains a raw sample tap. She will look for this during her site visit on 9/17/09. Chris also mentioned she has not seen a UV light during any of her visits to the site( website indicates they use UV for disinfection). After further review of the file the UV light was never an approved disinfection device and previous surveys by John McEvoy stated that it should be removed or go through plan review to be approved. A note from Deborah Hoy on 7/7/06 indicates that the UV light was removed but not verified by our staff. I could not find any indication in the file that disinfection was required for this system. ACTION NEEDED: none
