Details:	SUMMARY: Plan Review 213-2004, Final Approval DETAILS: Final Approval granted for Well L72116 which was completed on July 17, 2004. The DWP assigned the plan review number and waived the plan review fee ($150) in 2004. A site evaluation and well construction evaluation was completed by Bob Maynard, WRD well inspector on September 27, 2004. The Malheur County Health Dept completed a water system survey in November 2009. ACTION NEEDED: DWP to close out plan review.
