Details: SUMMARY: Nitrate alert DETAILS: The 4th quarter nitrate exceeded action level 6.68 for the 4th time. Contacted state for further sampling guidance. Require two more quarterly samples starting 2nd quarter April - June and 3rd quarter July - September. Will reevaluate at that time. ACTION NEEDED: Follow sample results and go over any corrective actions needed for the well.
