Details: SUMMARY: Significant deficiencies noted on 10/25/12 system survey DETAILS: Sent letter saying thirty days have passed since the system survey report was sent to the operator. Our office has not been consulted about corrections noted on the survey. Reminded her of the 18 week correction time frame. ACTION NEEDED: Significant deficiencies need to be corrected by 3/6/13
