Details:	SUMMARY: System Reactivation - Plan Review Requirements DETAILS: John called this morning to inquire about requirements to reactivate this PWS. They wish to lease the facility to a charter school. Since the PWS has been inactive for greater than one year - they need to demonstrate compliance with current regulations through the plan review process. I notified John that he would need to submit plan review fee ($150), schematic showing location of well, distance to any sanitary hazards within 100 ft, distance to surface water within 500 ft, well log, and current analytical results for water quality from well (coliform, IOC, VOC, SOC, Nitrate, Nitrite, Radionuclides, and Lead/Copper). John indicated he would be submitting plan review hopefully by end of week. ACTION NEEDED: Wait for plan review submittal.
