Details:	SUMMARY: Significant deficency corrective action plan requested DETAILS: I had e-mail correspondance with Marjorie Smith on December 13th. This was the significant deficency correction date from her survey conducted in June. Marjorie has requested in writing an extension to correct the deficiencies. I granted her a six month extension to develop a coliform sampling plan , develop and mainatin an operations manual and develop a plan to test and document chlorine residuals at least two times per week. Her new corrective action date will be 6/13/13. ACTION NEEDED: Check in with Marjorie in March to see how she is doing with implimenting the plan.
