Details:	SUMMARY: PB & CU SAMPLE SITE CHANGE INQUIRY DETAILS: NAN BIGEJ CONTACTED MY OFFICE TO SEE HOW THEY CAN CORRECTLY CHANGE SAMPLING SITES FOR TWO OF THEIR PB & CU SAMPLES. ONE SITE IS NOW A VACANT HOME AND THE OTHER IS A HOME WITH A PERSON OF LIMITED MOBILITY WHO MAY NOT BE ABLE TO FLUSH THEIR SYSTEM AS NEEDED FOR THE SAMPLING REQUIREMENTS ACTION NEEDED: COMPLETE THE FORM "SAMPLING SITE IDENTIFICATION AND CERTIFICATION , CHANGE IN SAMPLING STATUS" (141-A) AND INCLUDE THE FORM WITH THE PB & CU SAMPLES GIVEN TO THE ACCREDITED WATER TESTING LABORATORY.
