Details:	SUMMARY: Regulatory Assistance DETAILS: Contacted Cody Ott to determine what corrective action has been completed for the significant deficiencies listed in the sanitary survey of 4/30/15. Mr. Ott stated he did not recall receiving the sanitary survey report. He looked for the mailing during our phone conversation and stated he found the mailing and it was still unopened. I told Mr. Ott it was critical to begin required sampling and return to schedule. Mr. Ott stated he would submit water samples for coliform and nitrate within one week. I told Mr. Ott to read through the report and I would follow up with him in 2 weeks to discuss corrective action on the remaining significant deficiencies. ACTION NEEDED: Complete corrective action on all significant deficiencies.
