Details:	SUMMARY: Significant deficiencies and public notice for Sept 2015 GWR Violation DETAILS: I sent an e-mail to Alan Mustain to get an update on a request sent Dec 16, 2015 for a copy of any public notices issued for not meeting 4-log disinfection requirements in Sept 2015 and to determine if any of the significant deficiencies from the June 2015 survey had been resolved. Alan Mustain called to report that they will send in a corrected report indicating that they did not have a violation in Sept 2015 and therefore did not need to issue a public notice. This corrected report was received on 1/13/16 and the violation was rejected - no public notice is needed. Alan also said that they would work on getting the significant deficiencies resolved. ACTION NEEDED: City to respond to the status of correction of the signficant deficiencies identified in the June 15, 2015 water system survey conducted by Yamhill County.
