Details:	SUMMARY: Water System Survey Significant Deficiency and Rule Violations Follow-up DETAILS: LEPH was contacted by Mr. Warberg to inform us that he mailed and/or faxed the 2014 CC ASR and 2014 CCR today. He also hopes to have the O&M manual finished by the due date. As a precaution he is requesting a four week extension. ACTION NEEDED: Water system is to complete the O&M manual by June 10, 2016.
