Details: SUMMARY: Corrections for Significant Deficiencies From Survey (8/7/2017) DETAILS: Corrections for significant deficiencies from survey that was completed on August 7, 2017.The significant deficiencies and rule violations noted are as follows:1. The emergency response plan has not been created for this system (OAR 333- 0064(1).2. Chlorine ppm for residual maintenance is not tested and recorded twice weekly. Keep a log twice a week of ppm chlorine in distribution system (OAR 333-0036(5)(a/b)).In addition the following comments and recommendations were made at the time of the survey:1. Update the coliform sampling plan including a brief narrative of your water system and a map of the distribution.2. The treatment house is in need of repairs. Repair or replace housing in order to protect your equipment from damage. Damages could be done from pests, humans or the elements.3. Well housing needs to be secured. To minimize the potential risk of someone contaminating your water supply your well house should be secured. ACTION NEEDED: Corrections for significant deficiences have been completed.1. Emergency response plan was completed and proof of completion form filled out. See separate attachment for copy.2. Clorine residual is required to be taken by staff who is managing the facility during time of operation.For recommendations:A new building for the treatment housing was recommended as old building is not structurally sound. It was also advised that the operator contact the building department prior to putting in a new building.Coliform sampling plan should still be updated and the sites that are listed on the sampling plan should be followed for taking the required monthly samples.The well house was missing the door. That should be reattached.
