There are no significant deficiencies:

The recommendations and comments noted are as follows:

1. The DWP has established criteria for determining whether a system should be considered to have "outstanding performance." Systems that are
designated outstanding performers may have their water system survey frequency reduced from every 3 years to every 5 years. Although your water
system did not meet the established criteria, please review the enclosed handout to see what steps you can take in the future towards receiving
this designation.

2. Report ALL bacteriological and chemical results in exceedance of the MCL to this office within 24 hours of notification from your laboratory. At
that time we will assist you with repeat sampling procedures, corrective measures, and information regarding public notification.

3. Notify all users by issuing public notice whenever a contaminant level has been exceeded or a reporting requirement has not been met. The notice
must be posted or hand delivered to ensure that each user is aware of the MCL exceedance or the monitoring and reporting violation. The public
notice template can be obtained from http://oregon/gov/DHS/ph/dwp/tools.shtml or by contacting our office. Please provide us with a copy of the notice
for our records.

4. A summary of your monitoring requirements can be found on page 8. Please maintain a copy of this page and refer to it for future monitoring scheduling.
