Details:	The operator has been informed of the Arsenic sample submitted on 9/15. This is the initial Arsenic sample that was never submitted for plan review, prior to approval. This satisfies the deficiency noted in the 2022 survey report. The operator had been in communication with me about this sample and possible treatment options and installers. He will be notifying all staff persons of this situation similar to Green Spring Inn.
