Details:	SUMMARY: Corrective Action Plan has been established to correct the significant deficiency to the March 5, 2014 water system survey. DETAILS: 7-21-14 --- Received input from Joe Carlson, Chuck Michael, and Russ Kazmierczak regarding the corrective action plan for the one significant deficiency noted on the March 5, 2014 water system survey. The deficiency is for failing to meet setbacks from wells to hazards. Source assessment sampling has been completed by the water system, but DWS is requiring MPA sampling as well. The deadline for collecting the two MPA samples is June 30, 2016. ACTION NEEDED: See above.
