Details: | Rick informed me that Coos Bay-North Bend Water hired a contractor to replace an old water main. The new main was installed on south 8th Street from Ingersol Avenue south to Kruse Avenue. The west side of the project was then tied into the existing distribution system and all customer service were connected to the new water main. On 10.16.2023 the final tie into the existing water distribution system was performed following best management practices. A bacteriological sample was collected and tested on 10.17.2023. Test results showed presence of TC and absence of E. coli. The sample result was relayed from one employee to another via voice mail. Employee one did not realize that employee two was on vacation. Management became aware of the positive TC on 11.02.2023 and proceeded to collect an additional sample from the affected area. Lab test results for the new water sample confirmed TC positive and absence of E. coli. The general manger, Ivan Thomas was notified by Rick on 11.02.2023 and corrective actions were initiated. Staff flushed different areas of concern along the new main water line. A boil water order was issued to approximately 50 customers on south 8th Street from Johnson Avenue to Kruse Avenue, between 7th street and 10th Street. No eateries were affected. Language access information was not needed for non-English speaking customers. Lifted 11/06/2023: Rick informed me that they have conducted extensive flushing and chlorine testing in the area of South 8th street. This was followed by bacteriological samples taken on the 2nd, 3rd and 4th of November 2023. All samples came back Negative to TC and E. coli. On 11.05.2023, at 8 am, customers previously on the boil order notice were notified in person or by door hangers that the Boil Order was now lifted. Ivan Thomas, water system manger, informed me that they are taking actions to prevent this from happening in the future. Employees are now required to follow an d notification protocol until management can meet to revamp their communication SOPs. Employees are now required to do in-person notification of test results. |
