Details: The operator reported that they issued a boil water advisory due to a 6” main break near the intersection of 16th Avenue and Applewood Drive. The main was completely shut down to mitigate property damage and complete the repair. Date and time of break: 10:45 AM on 7-29-2025. Location: On16th Ave from 17th Ave to Coos River Hwy., Cedar Dr, Bayview Dr., Applewood Dr., and Coos River Highway from 16th Ave. to Lillian Slough. Approximate Number of Services affected: 85. Expected Duration: 7-29-2025 at 12:00 PM to 7-31-2025 at 12:00 PM. Notification: Direct notification. No language access material required. Notification to customers included a map delineating the boundaries of the affected area. No food services, or other OHA licensed facilities affected. County EH program manager notified. The boil water advisory will be lifted once repairs have been completed, water pressure restored, and after lab results for water samples confirm absence of bacterial contamination.

Update 07/31/2025:  The operator reported that the second sample collected along the Coos River Hwy came back positive for Total Coliforms and negative for E. coli. All samples collected after the leak repair were Absent/Absent. The system is now resampling the Coos River Hwy this morning and Friday morning and extending the Boil Order notice in that area until Saturday at 12:00 pm. The boil advisory for Coos River Hwy from 16th Ave to Lillian Slough will stay in effect until Saturday, August 2nd, at 12:00 PM. The Boil Order has been lifted for the following: On 16th Ave from 17th Ave to Coos River Hwy Cedar Drive Bayview Drive, and Applewood Drive.

Lifted 08/02/2025:  After 2 consecutive lab results showing absence of bacterial contamination, the boil water advisory was lifted on Saturday at 12 PM.
