| Details: | Wellhead was underwater during flood in Corvallis. Contacted operator and informed them their system is on a boil water notice due to the fact that the system was underwater. Emailed the Tier 1 Boil Water notice form and "Commonly asked questions boil notice" document.
4/30/19 Left a message to take the 5 confirmation samples and changed notice from voluntary to required from the wellhead. Lifted 06/05/2019: Coliform sample returned negative for coliform and E-coli. Boil notice can be removed. |
