Details:	04.26.2022-I emailed Daniel another copy of the final approval request form. 01.12.2022-Copy of Survey report and cover letter sent to Daniel. Site visit was done on 11.18.2022. Deficiencies-violations must be corrected by 03.02.2022. System completed the installation of surface water treatment in 2019. Treatment includes UV. Final project approval still pending. Daniel has to complete the final approval request form and email it to Rebecca Templin. The form was provided during the site visit.
