| Details: | I contacted the system to remind them that the corrective action due date for the significant deficiency for no plan review approval is 2/22/22. I referenced the items listed in the conditional approval letter sent to the system on 12/21/21 and Ms. Grossman thought that they had already been met. I asked her to contact OHA DWS to discuss it. She was granted an extension until 3/22/22 with the agreement that she will contact Carrie Gentry with OHA DWS and provide her with what is needed to grant final approval. | ||
