| Details: | 02.28.2022-Cal informed me that the report was mislabeled and the wrong report was sent. he re-sent the correct report today. No information provided on status of survey deficiencies. I sent another email to Cal inquiring about survey deficiencies. 02.25.2022-I emailed Cal enquiring about the reasons for the recent violation generated for late-nonreporting of SWTR report for January, 2022. I also asked Cal to provide an update on the status of the corrective actions to address the survey deficiencies. |
