| Details: | Contacted Pastor Lyle regarding samples taken on March 20, 2018 that should have been label temporary routines not routine. Nielsen Labs have been contacted and will be making the appropriate changes. I also mentioned that a Level 1 Investigation form had been sent to him. He was not aware of this. I resent this form in an email and he is to fill out and send it to me ASAP. |
