| Details: | Contacted Sam Watson multiple times (phone and email) to inform him that the significant deficiency corrections from the water system survey completed on July 22, 2021 are due by February 22, 2022. On February 24, 2022, Mr. Watson said during a phone call he would send me a correction action plan the next day. Mr. Watson was sent another email reminder on March 7, 2022. Mike Watson indicated in a phone call then in a follow-up email that on April 5, 2022, that the significant deficiencies will be resolved by June 1, 2022. As requested, guidance documents and templates for the significant deficiencies were sent to the Mike and Sam Watson May 18, 2022. Called on May 31, 2022, and reminded Sam Watson that the system needed to send confirmation that the significant deficiencies needed to be resolved or request for extension needed to be submitted by June 1, 2022. |
