| Details: | Dan Rocha called our office on 5/6/2022 at 11:00 am to discuss the results of the three(3) repeat samples. Two of the repeat samples came back positive for total coliform. I emailed over the Level 1 investigation form to Doris Rocha and informed Dan that this will need to be completed within 30 days and a corrective action in place. The operator stated that the area near the sample sites has been overgrown with Ivy and they took the samples while it was raining. Once they remove the Ivy, they will take a special sample to assure the system is clear of TC. Operator stated they will get the Level 1 investigation form back to our office next week sometime. Operator turned in Level 1 investigation form into our office on 5/16/2022. I sent the L1 investigation form into DMCE on 5/17/2022. Doris called me on 6/13/2022 to let me know the special sample taken on 6/2/2022 was clean ( no coliform). |
