Details:	11.29.2022-Daniel emailed me today, informing that all pending survey deficiencies will be completed on 11/30/22. A template for the protocol for under-certified Op was provided via email. 11.15.2022-I emailed Daniel requesting an update on the deficiencies still pending: Cross connection, Op and Maintenance manual, ERP and a Protocol for under-certified op. Monitoring requirements corrected on 03.28.2022. 05.25.2022-Daniel contacted Jay. The plan is to make a custom piece and install it. A box tubing (square cross-section) with a gap cut along the bottom. The due date to resolve the cross connection deficiency has been extended to November 30, 2022. 05.21.2022-Daniel Informed me that he took care of the cross connection violation. He emailed me pictures showing that he installed a piece of stainless steel in between the flanges to create a physical stop. I informed Daniel that after consulting with our back flow prevention specialist, his solution is not acceptable. There has to be an air gap. Daniel later clarified in an email that the reason he is avoiding the air gap is to keep the header intact because it is pretty much self supporting. With the current clamp style compression fitting used, he needs the cross tie to hold it together. As an alternate solution, Daniel suggested replacing the gaskets on the horizontal connections with a washer between the flanges so that there is effectively a very small air gap. I consulted with our plan review engineer and this solution would also not be acceptable. Daniel was advised to contact Jay Macpherson to discuss other alternatives.
