| Details: | All 3 repeats and triggered samples TC+ following 8/23 routine positive. Called Butch and he had already shock chlorinated the well with chlorine and found a vent plug in the well head missing. Sent Butch Level 1 investigation form and asked to complete and submit asap no later than 30 days. Continued routine sampling. |
