| Details: | Contacted Karl Ekstrom by phone at 8:35am on 9/14/22. Informed Karl that the repeat samples came back positive for total coliform and that this will generate a Level 1 investigation. Operator said that nothing strange has happened with the water system and that the spring flow often decreases in the fall, triggering positive results. Sent Karl a copy of the Level 1 investigation form and letter from the state over email at 8:41am on 9/14/22. Received Level 1 Investigation report from Grand Ronde. Submitted it to DMCE on 10/4/2022. Karl stated in report that low spring flow may have caused the TC+ sample due patterns from prior years. Corrective Action 9/28/2022 - GR stated nothing at this time. - CT |
