| Details: | Deficiencies noted during 7/22/21 survey. Reservoir hatch not sealed and late lead and copper sampling. Have been working with system and lab for over one year without results. Also, no certified operator. Discussed in field on 9//1/22 and emailed several times in last 12 months. Will send survey deficiency letter. |
