Details:	Spoke with operator on 11/11 and 11/14 regarding the three repeat and one trigger sample required following the positive routine sample from 11/8. She reports that over the weekend she did super chlorinated the storage tank, I mentioned to her to inspect the storage tank to ensure that there are no obvious opening including the screened vent. She has been informed that 3 temporary routine samples are due by the end of the month. I requested that she send me the completed Level 1 Investigation form by the 23rd due to the upcoming holidays.
