| Details: | I spoke with the operator of this system about the three unresolved significant deficiencies noted during the May 2022 water system survey. 1. The operator stated that a direct responsible charge has attended the small water system operator course but that the certificate has not yet been received. I intend to send links to the operator certification application to the operator. The operator will use this form to notify OHA of the certified direct responsible charge. 2. The operator stated that the sanitary seal of the well has been closed with flex seal. A confirmation photo will be sent and evaluated. If the solution appears sufficient. The deficiency will be noted as corrected. 3. The operator stated that the fuel tanks within required setbacks from the wellhead has not been moved. However, a new concrete pad has been poured to relocate this device and the operator will report back the projected move date, a map of the planned location and follow-up with a photo showing the completion of the move. |
