Details:	Phone contact with operator regarding deficiencies for system. System has CSP, ERP, and O&M with policy for under certified operators. System is continuing to work with plan review team and will follow up on set back hazards around well and vent screen. System noted sending in DRC information to OHA. System has sent picture confirming disconnection from "new well" from water system. System has competed all deficiencies associated with survey.
