| Details: | Spoke with Jacob Holden about correcting an overdue significant deficiency that was identified in the survey conducted 5/18/22. I had sent out they survey follow up letter and deficiency follow up letter to the water system operator on 2/27/23 but never heard back. I also sent those documents to Mr. Holden today via email. | ||
