| Details: | Background/Problem Identification: Arsenic was detected in well so entire system is under DO NOT DRINK notice. Assistance Provided: Met with operator and contractor on site to discuss problems. Advised that a well sample should be taken to isolate the well. Created a schematic of a proposed new treatment system with storage tanks to address problem. Owner to consult licensed professional engineers for design system for plan review with OHA.
