| Details: | 05.11.2023-Alisha contacted me and provided photos verifying the corrective actions for the following survey deficiencies: - Screens on storage tank overflow and vent installed on 04.28.2023. - Operation and Maintenance manual updated - resolved on 05.11.2023 - ERPA updated - 05.11.2023 - Coliform sampling plan updated - 05.11.2023 Next steps: to address the survey deficiency associated with the CT values not being calculated correctly, the system is working with Civil West to conduct a tracer study. Alisha requested additional time to correct this deficiency. Time extension granted. new corrective action due date pending. Alisha will email me back once she hears back from Civil West. |
