| Details: | I sent an email to NW Hazelnut notifying them that their significant deficiency for having an operator with a small water system operator credential has not yet been fulfilled. An operator has gone to the class but the form notifying the state has been in process for a few months. I attached a deficiency letter and sent one by paper mail. |
