| Details: | Contacted Karl by email at 4:30pm on 9/28/23. Forwarded over TC+ results from the triggered samples. Told Karl we will continue forward with the Level 2 investigation as discussed on 9/26/23. Conducted a consultation L2 investigation 10/16/2023. Last on-site investigation occurred less than a year ago on 11/21/2022.Operator is unsure of the source of contamination. Operator is working with an engineer to install disinfection as the corrective action. - CT |
