| Details: | No formal contact established with operator following 'Alert'. Attempts to contact include: Alert recieved 4:18 pm, 10/9/23. Emailed Paul at 4:46 pm, 10/9/23. Called Paul at 8:13 am, 10/10/23. Left written talking points with EH Support Staff (due to being unavailable for the remainder of 10/10/23). Called Erich at 1:40 pm, 10/11/23. Erich called back at 2 pm, 1/11/23. He stated that he would go check on Paul, his neighbor. As of 10/16/23, we have not heard back from the operator regarding this issue. We will continue trying to open a dialog with the operator to schedule a survey, resolve the Total Coliform positive, and address the outstanding CCR violation concern as well. 10/18/23: OHA-DWS sent notice that a Level 1 investigation was required (via email to HRCHD, and via snail mail to operator). We forwarded the notification directly to the operator (via email), and to the back-up operator contact person (Eric, via email). |
