| Details: | Scott called on 10/27/23 about a violation card received, In response the following email message was sent: Scott, This is my suggestion: You might want to ask your lab for help, but Find the lab result submitted to the state for the sample taken from the well on 10/4/23. In a clean place on the lab report, make a nicely written statement saying, THE WELL SOURCE FOR THIS SAMPLE IS AB. RECORDING AA AS THE SOURCE WAS DONE IN ERROR BECAUSE [and in few words explain why the mistake was made]. PLEASE CORRECT THE DATA BASE. Also find the lab report submitted to the state for the sample taken from the well on 10/10/23. In a clean place on the lab report, make a written statement saying, THIS SAMPLE WAS MEANT TO BE TAKEN AS A TRIGGERED SOURCE SAMPLE. THE LAB SLIP WAS MARKED AS A REPEAT SAMPLE IN ERROR BECAUSE [provide an explanation why the mistake was made]. PLEASE CORRECT THE DATA BASE. PLEASE REMOVE THE VIOLATION CAUSED FOR FAILURE TO SAMPLE THE SOURCE. You or your lab can then send these 2 documents to dwp.dmce@odhsoha.oregon.gov There is no guarantee that Ocean Pines will get credit. But you can ask. |
