| Details: | NCPHD left a voicemail for John for the follow-up about the deficiency from 4/17/23 for a certified operator and the due date of 12/13/2023. John called back and informed NCPHD that he currently has taken the training, but has to call the exam proctor to schedule the exam. He will call NCPHD to let us know when he'll take the exam. |
