Details:	Mr. Nix contacted me on this day and informed me of several violations related to assessment monitoring. After informing DWS, several violations have been removed, and assessment monitoring for 12 months has been satisfied and quarterly monitoring for coliform resumes beginning January 2019. In addition, the system had been on 4-log monitoring for frequent TC+ (2010-2016) as well as E. Coli positives in 2014 and 2016. The system underwent reconstruction of the wellhead in the spring of 2016, has performed successful assessment monitoring, and therefore has now completed the correction of all deficiencies. The submission of plans related to the deficiencies is not online with DWS.
