| Details: | The monitoring and reporting form utilized by Monroe for December 2023 is the incorrect form. The correct form was provided to the water system prior to the site visit on 21 November 2023 and November 22, 2023. The correct form was again provided to the water system on 18 January 2024. OHA-DWS has not received any of the information requested on the day of the site visit and in the email from November 22, 2023. |
