Details:	I went to Addivia on 01/07/2019 to look at the recently installed hypochlorite equipment. Mary Lou met the conditions stated in the plan review letter PR#155-2018. She has a DPD-test kit, handwritten procedures for checking chlorine levels, a raw sample tap, and a treated sample tap. She also fulfilled the corrective action for the recent Level 2 investigation. Sipple Well Drilling Company came out and fixed the leak at well #1. .
