Details: | The operator has submitted a request for an extension on correcting the deficiencies and has provided a written corrective action plan that notes the following: "Finished Water Storage - Hatch not locked or adequately secured. This hatch is at the top of one of the reservoirs at the boardroom location. To make this correction the Association to plans to hire a design consultant to design a fall arrest system for staff to use on top of this tank. The tank top is sloped and unsafe for anyone to be on without being connected to some sort of fall arrestor. This has been brought to the boards attention and will be further discussed at the March 19 th board meeting. We are requesting an extension on this through July 31, 2024, possibly longer depending on a designer recommendation for the fall arrestor system. Well Construction – No Screen on existing air vents. All three existing wells will need to have new holes drilled to install screen air vents. Assessment and completion of this work will need to be done by a well expert/driller. The association will need to hire the appropriate contractor to get this work done. We would ask for an extension with a completion date of June 30, 2024. Well Construction – Does not meet set back from hazards. This deficiency was identified by OHA from a system survey conducted August 8, 2020. The association applied for a variance as the well does not meet construction standards. The variance was denied by OHA. Oha did notify us that the use of this well was approved to be used as a source until such time a source water sample from this well came back positive. It should also be noted the well is leased and not under ownership of Orchard Heights Water Association. The association is in the process of new well construction at a different location. When the new well is constructed this well will likely be discontinued." |
