Details: 2/28/24 survey noted no ASR and well head possible below rim of vault. ASR submitted 3/20/2024 and Nyle sent photo showing stem wall of vault is lower than top of well casing and they will be installing a pipe with screen to ensure water would drain from vault before reaching top of well. These deficiencies are considered corrected.
