Details: Phone contact with operator, operator noted possible damage to well head is being investigated and source sample was taken. no routine samples were taken or may not be processed yet. System will continue to take annual assessment samples and ensure UV system is functioning properly.

System resampled in distribution and source after assessment sample. distribution samples are absent for bacteria, source sample is present for bacteria. System will continue quarterly monitoring in distribution and annual source sample.
