| Details: | Phone contact with operator, operator will have three repeat tests taken in the distribution and one source sample after routine distribution sample is present for bacteria. Discussed checking UV system. System noted bacterial sample may be related to repair to Arsenic treatment. Discussed source assessment sample system will continue to sample annual for source assessment monitoring. System did not sample for repeats as required and has been issued a level 1 investigation and violation for no repeat samples. System will provide level 1 investigation with-in 30 days to LCEH. Late repeat samples reported 5/20. All repeat samples are present for bacteria, source is absent. Facility will continue to sample monthly. |
