| Details: | Richard Gardiner left a message following receipt of significant deficiency warning letter on 1/15/2019 indicating that he had not received the original survey report. I scanned a copy of survey report and sent it to him via email. The original survey report was sent to Suzanne Herron listed as the administrative contact for the water system on 1/24/2018. Richard indicated via email that he should be the administrative contact not Suzanne. | ||
