| Details: | Mr. Conway, an operator for this system, contacted me on 8/3/24 informing me of the repeat positive sample for this system. I provided two options for him including just flushing the system or shock chlorination. I highly recommended shocking the system but proposed a flush if he was unable to perform the shock. I am providing additional instructions on the shock process to him today 8/6/24. I also am providing him with the required level 1 investigation form. |
