| Details: | May 15, 2024 survey noted an open bolt hole in top of wellhead. Dave sent me pictures of correction on 5/29/24. Hole was filled with steel wool and also acts as casing vent. System still needs to submit missing CCR. Certification forms submitted, but no copy of CCR. | ||
