| Details: | A letter was sent to the operator (8/14/24). We also phoned the operator and left a voice-message (8/14/24). In part, the letter said: "...This letter is a notification that multiple monitoring and reporting (water testing) violations have been assessed by the Oregon Health Authority, at the ‘Lava Bed Orchards’ public water system (#4195609). Most recently, a test for microbiological contaminates (coliform bacteria) was missed during the month of July, and a quarterly Nitrate water test was missed for the second calendar quarter (April-June). Please return to compliance as soon as possible. Also note that there are additional testing deadlines quickly approaching..."
